Scott Monson, LADC, Mdiv
Clinical Program Director
Scott Monson was hired as Assistant Program Director in November 2012 and promoted to Clinical Program Director in May of 2013. Scott has this to say about his work at Transitions:
“I’m proud to be part of a team that offers personal, individualized care to people in recovery. Transitions provides services that blend tried-and-true recovery practices with the best new research in the field. It’s extremely rewarding for me to see our clients heal, grow and embrace their new lives in recovery as they progress through the Transitions continuum of recovery programs. “
Scott brings considerable training and experience to Transitions. He has a Bachelor’s Degree from Minnesota State University–Mankato, a Master of Divinity Degree from Seabury-Western Theological Seminary in Evanston, IL, and a Graduate Certificate in Addiction Studies from the University of Minnesota–Twin Cities. In addition to his work as a Licensed Alcohol and Drug Counselor, Scott serves as a priest in the Episcopal Church and teaches in the Integrated Behavioral Health and Addictions Studies Programs at the University of Minnesota.

Sam Woolery
Cornerstone Program Manager
Sam Woolery was hired as Manager of the Cornerstone Program in June of 2010. He brings boundless enthusiasm and dedication to a role that truly includes a little bit of everything. As manager, Sam works hard to make Cornerstone a safe, encouraging, and fun sober living environment while ensuring residents are accountable to house expectations designed to promote an investment in the recovery process. Sam also schedules regular social gatherings that allow current Cornerstone residents to connect and form friendships with numerous program Alumni who are continuing to find success in both recovery and life, often years after their Cornerstone Experience.
“The best part about my job at Transitions is getting to have unique, impactful conversations with residents every day. I live on the top level of Cornerstone house, which allows me to be a consistent presence and source of support. I share my experiences and invite them into my life so they will feel more comfortable inviting me into theirs.”
Sam was born and raised in Sedalia, Missouri and studied at the University of Missouri-Columbia. His path to Saint Paul, Minnesota began Thanksgiving weekend, 2007, and is not unlike the path that led many to the program he manages. He enjoys all things baseball, writing poetry, and every Thanksgiving with his family.

Troy Mock
Foundation Program Manager
Troy Mock was hired on as Residential Manager for the GRH Foundation Program upon its opening of the new facility—Arches—in February, 2013.
Compassion and empathy are some of the strongest character traits Troy brings to the program, while still able to provide residents with solid structure. Individual independent living skills are one of the key building blocks Troy incorporates into his daily work with residents.
Troy was born and raised in Phoenix, Arizona and lived there until he decided he wanted a new path in life, free of alcohol and drugs. His recovery journey brought him to St. Paul, Minnesota in October of 2010 and he’s been in Minnesota ever since.
Troy graduated from Arizona State University with a B.S in Accountancy in May of 2002 and has personal work experience in the accountancy field as well as retail and health product sales. His hobbies include softball, watching all sports, going to movies and hanging with friends.
